SMART® Health Card Verifier App FAQs

  • The SMART® Health Card Verifier App allows someone to scan a SMART Health Card QR code to quickly verify an individual’s COVID-19 test or vaccination status. The service is brought to you by The Commons Project Foundation, built on standards from SMART Health IT.

  • When a SMART Health Card QR code is scanned by the SMART® Health Card Verifier app, two key pieces of information are displayed: (1) whether the SMART Health Card is valid and (2) whether the issuer is in The CommonTrust Network’s registry of trusted issuers. Other information displayed includes issuer name, vaccine or test type, dates of vaccine or test, and name and date of birth of the recipient.

The SMART Health Card Verifier App does not scan paper CDC cards.

  • The verifier app is designed for anyone who needs to be able to quickly verify individuals’ vaccination status in a secure and fraud resistant way. Free to use, the SMART® Health Card Verifier app makes verifying health status easy.

  • A SMART Health Card is a paper or digital version of your clinical information, such as vaccination history or test results. They allow individuals to keep a copy of their records on hand and easily share this information with others if they choose. SMART Health Cards contain this information in a format that is secure, portable, and easy for entities like airlines, governments, and venues to verify. When scanned, a verifier can easily see that the information on the card has not been tampered with, making SMART Health Cards a safe and reliable way to demonstrate health status.

  • Simply download the SMART Health Card Verifier App from the Apple or Google Play store and it’s ready for use, without any need to log in. Use the camera within the app to scan SMART Health Card QR codes and one of the three results will appear: Verified, Not Verified, or Issuer Not Recognized.

  • This means that the SMART Health Card scanned is valid, but came from an issuer that is not in the CommonTrust Network’s registry of trusted issuers. This may indicate that the issuer is not part of the CommonTrust Network. In the case of an unrecognized issuer, the issuer URL will be provided.

  • This means that the SMART Health Card scanned is not valid, as it may have been corrupted or modified after it was issued.

  • Only key information needed for verification is shown when a SMART Health Card is scanned:

    • Issuer name

    • Vaccine or test type

    • Dates of vaccine or test

    • Name and date of birth of recipient

  • No account is needed to use the app. The app does not collect any personal data about you as the user of the app.

About the CommonTrust Network

  • The CommonTrust Network is a registry of trusted data sources that includes testing providers, vaccination providers, and public health registries.

  • CommonTrust Network verified means that the SMART Health Card was actually issued by the listed issuer (for example, Walmart or the State of California). It also means that the issuer is an authorized test or vaccination provider or a government operated registry of health information that is in the CommonTrust registry.

  • At this time, the verifier app only scans SMART Health Cards for COVID-19 test and vaccination records. If you try to scan a QR code that is NOT a SMART Health Card for a COVID-19 test or vaccination, the verifier app will tell you that it is invalid.

  • The Verifier app only scans SMART Health Cards. Printed vaccination records that do not include a QR code, including CDC cards, cannot be scanned by the app.
